Why is my account suspended?

    Dear Jotform,

    My account at admissionsolinfo@gmail.com has been suspended, and I wish to seek your kindness in reactivating it.

    Below is a screen shot of what I get when I try logging in.

    Hello.

    According to our admin logs, your form named Online Learners Dashboard Signup was marked as ''Phishing'', because it asks for sensitive data such as login details, and/or passwords. Any form that violates our Terms of Use will be deleted, and the form owner will be suspended. If you like, we can review your form and delete the guilty fields to reactivate your account. For this action, we need your consent.

    Hello. We've successfully removed the guilty form and re-activated your account as you've requested. Please don't forget that creating any form that violates the Terms of Use will get your account suspended again.
