The widget looks correct on the Jotform site while editing it, but once published and on the website the individual fields disappear

When I look at the screenshot and link you have added, I can see that in the link attached you have added three different fields in the form whereas in the screenshot attached there is a single field used.
Can you please share the direct URL of the form that you are referring to so accordingly we will look into to understand.
