CUSTOM FORM

We have a paper form we would like to create with Jot Form, can your team build it , if we provide a pdf sample?

If you are providing a PDF sample of the form you can import the form to create this and our system tries to get the form to look like the PDF. 

Go into your My Forms and select Create Form:

Select All Questions on one Page (Classic Form):

Then select Import Form and select the Import Fillable PDF Form:
