Our IP address is banned

    I sent numerous emails and no response for days, I used your form as a alternative to google forms due to it being cleaner. However during me practicing with it and about to become a paid member. My IP was blocked, my forms disabled and no help from support whatsoever. I am trying to understand what’s happening. 


    Form is disable on my site and this is how I was going to direct customers.

    Are you referring to the account acsirichard? If so, I see that the account has been suspended due to the Insurance forms that were suspicious. Since the account is registered using a public domain email address and the quotation forms were suspicious for phishing attempt, the account has been suspended. 

    Online Form Builders, including Jotform, are sometimes being used for identity theft and we go to great lengths to prevent this. That is why collecting sensitive information is considered as phishing activity, as stated in our Terms of Use.

    Our phishing detector will automatically flag the forms found and suspend the account or this can be manually performed by our Staff.

    In order to reactivate your account, kindly provide us the following information:

    1. The URL where your form is used.

    2. Your website or proof of affiliation to a legitimate institution.

    3. Share with us a document that will prove that you're authorized to use the said logos.

    We also need you to change your registered email to a corporate one to avoid getting flagged again by our phishing detector.

    Send these proofs to reviewers@jotform.com with Thread #1923699 as your subject. We will let you know about the status of your account after a thorough review of your proofs.
