May we know what problem did you encounter when you tested your from from your website? I went to the link you provided to check your form. It seems that the iframe's current height is not enough since the submit button is either partially or totally hidden depending on the browser used.
I also tried to send a test submission from your website and I was able to send it successfully. The custom Thank You page was shown, and I found my entry in the form's submissions page inside your JotForm account. Your form is working on your website.
