My account has been suspended

Is it possible to reactivate my account and we will remove the account no. details? We have seen in the previous questions that it is one of the reasons why one account was being suspended.

These forms were found on your account as suspicious of phishing activity because you're collecting banking information, TIN and also using Copyright Logos. Following fields that triggered suspension of your account - 

1. Copyright logos - Your form has 2 copyright logos that you are not authorized to use. If you are associated with these businesses you need to send the documents to reviewers@jotform.com 

2. Bank account details - You cannot ask for this information if you are not a financial institution. 

3. TIN - This field also looks like it's captured user's personal identification number. 

We can lift the suspension of your account if you agree with our terms and allow us to delete the offending fields mentioned above.

logos, bank account details, TIN info fields in the form. I hope my account
will soon be reactivated.
