When I select cancel on my account I am taken to a page (plimus) that I do not have a username or password for. I do not want to be charged another month and i want to downgrade to the free option immediately.

I can see from our database that you have already initiated the request to downgrade your JotForm account. It will take effect on 2011-04-25.
You may not need it anymore, but your Plimus username is the long string of numbers from the original Plimus email you received when you paid for your JotForm account.
