The link I've been given has Elements not on the form I built & things are in the wrong order?

I've confirmed that the built & pdf/downloaded versions are correct, but when I use the web link version it is showing elements that are not built into the form and the elements I've added are in the wrong order?

For instance:

- Patient Birth Date (I already have Date of Birth above it)

- Subscriber Birth Date (I don't want this)

- Several other items are all out of order?

I've cloned this form https://form.jotform.co/92257423115856, but I don't see the Patient Birth date and Subscriber Birth date field.

Do you still need help with that form? If it's a different link, please share that link so we can check.
