You might be viewing it on IE browsers, I'd recommend using updated browsers like Firefox and Chrome for now while our dev team are working for a fix on IE browser related issues with our Form Templates page.

Tested in WinXP IE7,8,9 and Win7 IE8,9 all working on my tests at browserstack. Could you please try again?
