How to prepopulate the form fields?

    I'm a truck driver. My employer has a Driver Referral program, and they provide a form of their own for this purpose. Please click this link https://drivers.jbhunt.com/driver-referrals to view the form. It is not the form I want to use.

    As you will see, the form asks for my Name and Alpha Code. I would have to manually input that info for each applicant, and I don't want to do that. And I don't believe their form would work well with social media.

    I want to use Social Media to circulate My form with my info already embedded into it, but visible only to the recipient, and Not visible to the applicant.

    What do you got that will help me?

    I am not sure I understood your concern completely. Do you wish to prepopulate the Name and Alpha code fields? If yes, you can use the prepopulate app so the fields will already be filled when the user opens the form.

    Could you please provide more information about this? Do you wish to hide that information from the users who are filling the form? If yes, you can just hide those fields using this guide: How-to-Hide-Form-Fields
