Why is my account suspended?

    Dear Jotform,


    I have been using jotform for some time for security related forms at Komedia in Bath.


    I've recently updated some forms and since my account has been locked out, suspected pf phishing according to a web page display.

    Please advise ASAP.

    Hey there, Ian.

    Looks like one of your forms include sensitive information, which is against our Terms of Use here at JotForm. Your account was suspended because of this. Let's take a look at what specifically caused your suspension:

    Insurance numbers, passwords, usernames, passport details and so on are considered as sensitive information. Here's a the part of the Terms of Use which explains this:

    Phishing. You agree that Jotform, Inc. may terminate your Jotform Service immediately if a form's purpose is found to be deceptively obtaining, for example: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately.

    Since you can't access your account anymore, we can delete the guilty fields in the form and unsuspend your account. This will require your permission.

    We are waiting for your response to go on with the case.

    Please remove the offer using fields and reactivate my account. I will review my forms and ensure there are no breaches of GDPR or any other sensitve information.

    Hey again.

    We've deleted the guilty fields (some bank account number fields as well) that we found problematic. You are also unsuspended. Please note that if you continue creating forms that conflict the terms of use, your account will be suspended once more.
