Your account got suspended due to violation of our terms of use: https://www.jotform.com/help/8-Terms-of-Use
"Phishing. You agree that JotForm, Inc. may terminate your JotForm Service immediately if a form's purpose is found to be deceptively obtaining, for example: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately."
Asking for login credentials is not allowed. If you want the account reactivated, we will have to delete those fields first.

I am not *deceptively* obtaining credentials, I am a freelancer asking a
new client to provide credentials so that I can log into their account and
do work.
The form explicitly asks which account they want to use, and asks them for
the credentials I can use to log in.
According to the "Phishing" section of your TOS, it is not a violation to
ask for login information, it is a violation to do so deceptively.
Can you elaborate on this?

Unfortunately, there is no exception, we do not allow collecting login credentials through our forms. You would need to get that information through other means, via email or phone call, but it is not possible through a jotform.

p.s. You may want to update your TOS to reflect that absolute policy,
because that's not how it reads, to me. To me it says you can't use JotForm
for phishing (good policy!) but it doesn't preclude asking for login
credentials (and other sensitive data) for legitimate reasons.
