Is there a way to view the entire form? Thanks!

Thanks for asking. Neither Free text elements nor headings appear in notifications unless they are manually added. However, those elements can be seen in the form's submissions page. While there, click the little gear icon on the upper right side to reveal a menu and then select Show Headers and Texts:

To get the free text elements to appear in notifications too, please do the following:

    1. Select all [Ctrl+A] of the text in one of the free text fields and copy it [Ctrl+C] to your computer's clipboard

    2. With the form loaded in the form builder, go to the Setup & Embed tab and click Email Alerts:

    3. When the Compose Email section loads, do the following:

    a) Click the row in the Question and Answer table under which the text is to appear and then click the Insert Row After button (indicated by the right arrow in the screenshot below)

    b) A new row will appear where you can paste [Ctrl+V] the text

    4. Do the same for all of the other free text fields and when done, click Finish to close the notification

    5. Save the form
