Why am I suspended?

While I am trying to login to the Jotforms, I get a message saying "Login Failed - Suspended". Do let me know what is the issue with my login

It seems that your form named ''On-Boarding Checklist'' was suspected for Phishing by our systems. Here's the guilty part of the form:

Phishing is strictly against our rules here at JotForm. Here's what our Terms of Use says for the matter:

Phishing. You agree that JotForm, Inc. may terminate your JotForm Service immediately if a form's purpose is found to be deceptively obtaining, for example, sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately.

Since your account is not accessible, we can delete the guilty fields on the form and unsuspend your account. However, we need your written permission to modify your forms.

What will the action be for the case? Do you need us to delete the guilty form fields and unsuspend the account?
