Submissions from an imported fillable pdf form :2019 Employee Health questionaire

    Hello i have spent the past 2 days editing a jotform template from an imported Fillable pdf. I was under the impression that the "submission" i would receive would be the actuall filled out PDF form . but this is not the case. it is giving me a regular JOTForm submission. i really need to be able to pull hte data onto the imported fillable pdf form. Please tell me how i can do this. THank you

    I am not sure if you are referring to the submission data you received in your email. If yes, an email will always show the default template unless you modify it. However, you should have received PDF attachment in the original format along with your email.

    Please confirm if you have received PDF attachment in the original format.

    As I check the form https://www.jotform.us/form/92385412565158, it appears that you have used PDF converter to import the PDF form. Please note that PDF converter converts the online version of form while it displays the actual PDF form to the user and the fields needs to be filled online. 

    If you want to use the actual PDF form to receive the submissions to JotForm, you may import the PDF form to JotForm using PDF editor which is different from PDF converter. This way, you may send the fillable PDF form to the users and receive the submissions normally. You may also set the options to send email notifications.

    Since the PDF form that you are using is having multiple columns with tables, I am not sure if it can be imported to JotForm or may need to create the PDF form from the online form.
