Is it possible to repeat multiple fields?

Hello,

I created a form where the user enters multiple fields of a questionnaire. There are many names in each pull down menu. At the bottom of the form, it says "schedule more dates" but I want the first 3 fields to repeat the user's name, section and sub info. It is the same person, with the same information but they need to only change the date and 1-2 other pieces of criteria that follow. I'm trying to avoid them going through the entire process again filling out the same information.  Help!
