How do I add the submit button and HIPAA badge in my form?

I have two questions

1. I dont see the hippa compliant badge on the form
2. How do I add a "submit" button

This is a PDF form that was imported in the PDF Editor. The submit button is supposed to be automatically added. As for the HIPAA badge, I don't think the PDF has that option. We cannot check HIPAA account so kindly share the PDF form to our support email at support@jotform.com so we can try the import process to our end for that form.

I'm not trying to create APDF and trying to create an online form
I did import the form from APDF
When I imported it did have the submit form as the bottom But somehow I
delete it and there does not seem to be an option to put it back
...

I see. In that case, you will have to create the form through the form builder because importing a PDF will only create the same PDF. As you could see, the steps are the same, but it is a PDF when you preview the form.
