Can't get one form to populate the next form

I've tried the colorful instructions, the black and white instructions, and still can't get three fields from one form to populate another.  I've spent all afternoon, and I give up.

All I want is for the event name, first name, and last name from my booking form to  populate the same fields in the design form.

I've tried viewing the source, using the unique fields in the build view, and the best I can get is for the Event Name to populate the second field.

Please, help!
