PDF Converter: How to add a form field and link it to the uploaded PDF form?

If I understood correcly you want to add a new form field to the form you imported and also show that field on your PDF form. 

Unfortunately, that is not possible. You can only add a new form filed to the online form. The PDF form cannot be changed.
