I have a few questions regarding how the condisions work;
Can you have more than one condition per form? If yes, is every condition evaluated separately and actions are taken no matter what some other condition says? Example
Condition 1 is true
Condition 2 is false
Condition 3 is true
Will both the action from condition 1 and 3 be made?
I have two different versions of problems with this. In one form, some of the actions are taken, some of the recipients are receiving their notifications, but others aren't, even when their conditions are matched.
The other version of the problem is where no actions are ever taken no matter how I fill out the form. Am very sure that the conditions are actually matched in these cases as well.
So baiscally, is there something that I might be missing here?

Thank you for using Jotform Support Forum.
If conditions have conflicts, chances is it would not work. For example, if you create a condition like c=a+b execute A and c=d+b execute B.
I would suggest checking the conditions you have created for possible conflict. If you could provide us which forms you are experiencing this issue, we could check it for you.

If you could have a look that'd be really helpful.
This form does not perform any of the actions:
This form complets the two last actions but not the first one:

Your form notification settings are correct. But I suggest you to try using email@example.com as your form notification Sender Email. Just like this one:
