form with different shipping and billing address

Is it possible to have a checkbox with the question; "is the shipping address different from the billing address?".

If the answer is "yes", and then show address fields?

If so? How do I do this?

To clarify, do you want to create an order or purchase form?

You may use conditions to hide your Billing address field and show only when the user selects yes in the option.

To set this up, you can add a new Address field and hide it.

Then add a single choice or a dropdown field to ask the question if the Billing has a different address and use the Show/Hide condition.
