How can I make an age verification form?

I'm wondering if you guys have a template for age verification. I can not find one, but the person who recommended you guys stated he's currently using one. His form included ID and Face verification.

We do have an age verification for our Date Picker fields.

Go in your Date Picker Properties go to limits>Age Verification.
