My apologies, but I am not sure if I can understand your request correctly. Are you looking for an option to populate form from google sheet? I'm afraid we do not have a google powered option for that, but forms can be prefilled from attached excel sheet: How-to-Use-the-Spreadsheet-to-Form-Widget
