Is there a reset submission data?

I'm building a camp medical form. Parents may have multiple kids from the same family and using the same email address. Each child needs their own medical form on file. 

It appears that cookies are saved and when they re click the link for a second child it saves the previous information for the first child. 

Is it possible for the form to reset once the submit button is clicked? 

You need to disable the Autofill feature of the form to fix the issue. Here's a screencast guide that you can follow:

1. Open the form.
2. Go to the Settings page.
3. Select the Form Settings tab.
4. Click the Show More Options button.
5. Disable the Continue Forms Later feature.
