Can you make my form available to edit

hi firstly thanks for activating my account once again. 

as per your suggestions i would like to edit the form and make it appropriate as per your terms and conditions, however to modify anything i would need access to it. 

Request you to kindly stabilize this issue so that i have access to my form and i can accomodate the needed changes to it. 

I have checked your account and it seems my colleague has lifted the suspension and kindly add the recommended fields to avoid getting suspended again.
