Certain fields on my form aren't showing up.

    Hello, 
    I am still unable to see the fields I add. I have gone in and added them then when I go to preview it disappears. I have done all that is suggested and it still isn't working. I have added to screen shots for you. One is the Edit side and the other is the preview side.

    I have added the Father's Name field to your form and have updated both the notification and the autoresponder emails. You can let us know what other fields you would like on your form and where, and we would be glad to assist you in adding them.
