Grab email address

We are going to be putting a form into an email. This email will be sent to multiple people. We do not want to have them enter their name, email etc... we just want it to be a simple one question multiple choice answer form. 

Is there a widget or any way to grab a persons email without them having to type it in?

Unfortunately, that is not possible. The user needs to fill the email field to be able to get such information.
