How to remove the Left header template name from Print Preview

Hello, when I try to do a Print Preview of my form located here, I see that the template name is listed, which is "Parental Consent and Release for Powerhouse Youth Ministries". Is there any way to remove this template name from my final form? Thank you!

Unfortunately, there's no option to remove that in the Form Builder. However, you can disable the Headers in the Print option. Here's how to do it in the Google Chrome Print page:
