How do I make the Form mobile responsive?

    I am trying to make this form responsive. Right now it only shows as mobile. Can you help me? https://teenage-driving-safety.myshopify.com/pages/parent-teen-safety-contract


    Also, rather than submitting the information to us, the completed form should be send to the user at the email address they have provided. Can you help me do this as well?

    Do yo want to align the fields all in a line as shown and also make it responsive in the phone as well. I noticed that the bottom part of the Form is not aligned as shown:

    Also, please note since you have hidden your submit button so you or either the user would not be able to receive the submissions as an email nor you would be able to receive the submissions on the submission page. None of the responses would be recorded. The print option would just print the page of the Form which the user has filled.
