Thanks for the update - looks to be fine. Any information on the cause of the outage / what is being done to prevent future outages? If we experience any other disruptions, we will need to move our form to another service.

We made a bad code release that caused this problem. We are currently writing automated browser tests that will catch such problems in the feature. All future releases will have to pass these tests.
