How can I get my account activated again?

    Your account has been suspended because you were collecting credit card information and selling insurance via several forms.

    In order for us to restore your account those forms (or guilty fields) have to be removed so your form(s) is compliant with our Terms of use.

    Here are the offending fields in some of your forms:

    Online Form Builders, including Jotform, are sometimes being used for identity theft and we go to great lengths to prevent this. That is why collecting these types of data is considered as phishing activity, as stated in our Terms of Use.

    Our phishing detector will automatically flag the forms found and suspend the account or this can be manually performed by our Staff.

    You may collect driver's license numbers if your purpose is not to collect these data from the general public, but from your own customers, you just need to comply with two things.

    1. Add the Terms & Conditions Widget (or the Short Scrollable Terms Widget) for your users to read and agree.

    2. Add a Signature Widget so your users can sign and agree to provide such sensitive information.

    However, we do not allow uploads of any sensitive personal information like passports, social security card, driver's license, credit/debit cards, bank statements, voided cheques, birth/marriage certificates, visas, and photo IDs.

    Also, a form offering loans/credit/insurances must be done from authorized bank or credit unions only with the company email address on their registered Jotform account.

    We will need you to provide us these proofs :

    1. The URL where your forms are used.

    2. Your website or proof of affiliation to a legitimate institution.

    Send all of it to reviewers@jotform.com with Thread #1967482 as your subject. After we review those documents, we will update you about the status of your account.

    We would appreciate your cooperation in this matter.

    cannot login to account).
    Also, just to clarify, we can use JotForms to collect information for
    potential new clients for insurance purposes?

    Yes, however, we still need to review your webpage and proof of affiliation to a legitimate institution.
