How can I link a field to the uploaded PDF

The upload of the form worked okay but the upper left was all 1 field. I have rebuilt the form but cannot link the outcome to the form on the right. How is that accomplished?

As of this moment, you can only add/remove fields on the form builder but there is no way to link them to the PDF.

However, please note that the feature is already been worked upon by our developers. Unfortunately, I could not give you an update as to when it will be released.

Unfortunately, I am unable to view the original setup upon PDF upload hence I could not suggest anything that might help, if there's any. You may try creating another form with the uploaded PDF without modifying it so we can see the initial issue and maybe suggest another path.
