Looks like I have a mix of the free account and the 2nd level ($9.95). However, I'm not charged anything per month. I need to be on the 2nd plan and start paying since I need the additional submissions. How can I upgrade?

I checked the account under this username and email address the
results shows that there is only 1 free account associated with it.
Were you using the same username and email address during the upgrade?
To upgrade login to your JotForm account then go to Account Settings
and click on 'Upgrade to Premium' .
If you encounter any problem with the upgrade let me know.

Have you already tried subscribing by going to this link? http://www.jotform.com/pricing/
If so, may I know the email address you used and did you select Paypal or entered your credit card (Plimus)?
So far I'm seeing your account as a free account and can't seem to find any payment orders via Plimus.
