I cannot open my account - suspended

    When I go to sign-in it says suspended and the form we create will no longer work.  Can you help me with that?  Not sure what happened.

    Our form reviewers suspended your account because of the logo you're using and your form also flagged as a scam. Please check our terms of use for more information: https://www.jotform.com/help/8-Terms-of-Use

    Please provide more information like your corporate email and website so we can investigate it further.

    I looked through terms of use and I am not sure what we are violating.  Here is our club's website - http://www.fishersrotary.org/.  We are using the form for our members to submit their application to join the club.  Rotary Club is a non-profit organization and the logo is our local chapters logo.  Please give me more details on how to un-suspend our account.

    You violate the Copyright Infringement.

    We understand this might not be the case with your form, yet we had to act according to the general rule to be in compliance with our Terms of use: Free accounts with non-corporate emails showing suspicious forms must be suspended immediately.

    Before we activate your account, please provide these following:

    - Share with us a document that will prove that you're authorized to use the said logos.

    - The website or proof of affiliation to a legitimate institution.

    - Your corporate email. Example only: staci@fishersrotary.org

    Or, we can just simply remove the Logo and we will activate your account immediately.

    We'll wait for your reply.

    Please remove the logo for now and I will get those items so we can add it back.  We need use of the form in the in between time.
