Why my account is suspended?

    Hi I've been having issues all week logging in. At first it said that my IP address had been blocked due to suspicious activity (even from other devices) so I sent off an enquiry regarding that. Today it said my password and username were incorrect so I reset my password which was successful, and now when I log in it says account suspended. Please help me figure out what's going wrong I urgently need access to my forms. Thank you

    Your account was suspended because your forms 92173691274866 and 92581387495876 were considered as phishing one as collecting Driver License uploads and bank information is a violation of our Terms of Use:

    Phishing. You agree that JotForm, Inc. may terminate your JotForm Service immediately if a form's purpose is found to be deceptively obtaining, for example: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately.

    Collecting Sensitive Information. You may not use the JotForm Service to collect certain types of sensitive information, including but not limited to, credit card information and any type of login credentials. You may collect some sensitive information such as social security numbers or driver’s license numbers, but you are required to use best security practices of JotForm, Inc. including SSL and Encrypted Forms features. You are solely responsible for compliance with any data protection and privacy laws and rules applicable to the sensitive information.

    If your purpose is not to collect this data from the general public, but from your own customers, then you need to comply with two things.

    1. Add the Terms & Conditions Widget for your users to read and agree.

    2. Add a Signature Widget so your users can sign and agree to provide such sensitive information.

    In other cases, our phishing detector will automatically flag the forms found and suspend the account or this can be manually performed by our Staff.

    We also need you to change your registered email to a corporate one to avoid getting flagged again by our phishing detector.

    Also, we will need to remove the guilty fields (which are collecting bank information) from your forms.

    We can only re-activate your account if you agree to add the required widgets and change your account email address.
