Create Health Questionnaire on Jotform

Hi,

Can I create my own health questionnaire with Jotform? I signed up with Wufoo but could not even get half my intake form input. Their system can only handle so many fields I guess. I also need an intake form I can integrate with Acuity.

Yes, you can create your own health questionnaire using Jotform. You can choose from our form templates or you can start from scratch.

But unfortunately, integration with Acuity Scheduling is not available at this moment. However, you may consider using Zapier to connect your JotForm with Acuity Scheduling.

Can I make changes to a form template or do I have to start from scratch?

Once you clicked the "Use Template" button, it will create a copy of that form in your account. And yes, you can edit it if you want a more personalized form.
