Form in instagram displays this page isn't available

I need help finding my original form I created. I posted the link on my IG instagram account and it was working properly until recently. It now says the link is broken and page is unavailable.

Can you please share the link that is posted on instagram since the form under hairbytheemeraldfoxx is active and does not have the same issue as the one you've posted.
