How do I add sections meant to capture information?

Hi there!

I'm creating a form meant to grab and collect info including things like height, current weight, desired weight lost, male/female/prefer not to say.

I don't know which items to use from the left-hand side that will do this?

You could try using the Short text entry field and set the validation to "Numeric" from the field's properties.

OR You can also use the Number field.

For the question "male/female/prefer not to say", you can use the Drop down field.
