Form not including all fields when submitted

    My Pre-Consultation Behavioral History Form has an aggression screen written into the form.  You can fill it out online and then submit the form, however, the aggression screen will not show up in the new pdf editor.  I'm missing this information when I download in the pdf edition.  Additionally, I can only save now in the pdf editor.  Help!!!


    Also, I had a client complain that the form timed out while she was filling it out.  She lost all of the information.  I had to send her a Microsoft copy of the form to fill out. If I'm having all these problems, I may cancel my subscription and go back to using Microsoft Word.  It's a real headache all these issues.  

    See the photo below for what I see filled out on the form:  Nothing even though the client filled out the information online.  It appeared correctly in the email copy of the completed form that I received.

    I just cloned your form, made a test submission, opened the PDF Editor and downloaded the PDF. I was able to see the Aggression Screen question and it's related response correctly in the downloaded PDF:

    I think users may be skipping that field in your form because that field is not mandatory. Would you mark that field as required and could you allow us to test submit your form to check this issue on your end?

    No they aren't skipping the field because it shows up in the email reminder
    I get when a form is submitted. I get a cc copy to my email and the
    aggression screen shows up. When I check it in the pdf editor, it does
    not.
    I did what you asked. I submitted a form and filled out the required
    fields and the aggression screen (as if I were the client). I submitted
    the form. I checked my email. The aggression screen showed up in the
    email; it showed up when I looked at the form in the "inbox" (original
    style). However, when I pdf'd the file and it went to the pdf editor, the
    aggression screen was left blank. This is a huge problem for me.
    Can you access the form I filled out this morning? It's the most recent
    addition to the inbox. If not, let me know how I can let you access it.

    Here is the link to the completed form. This is the mock form where I did
    fill out the aggression screen. However, in the pdf editor, it doesn't
    show up. See: https://link.jotform.com/NcrxiE9Vbf.
    I've attached a copy of the same form (saved from the inbox). Note: I'd
    like to be able to pdf a submission without automatically being taken to
    the editor.

    Hello - I was not able to replicate the issue that you are currently encountering. May I know which platform and browser you are using?  Try using another browser. Let us know how it goes on your end.

    I was using Google Chrome and I just tried it on Safari with the same issue.  Are you able to help with this issue because clearly it's happening on my end despite the browser I use.
