Inaccurate Ordering

I have used jotform a couple times for uniform ordering. 

I end up receiving inaccurate ordering information. 

A customer will submit their order form and order 3 short sleeve shirts, but on my end it might show up as LONG SLEEVE shirts. 

There have been several discrepancies like this.

Do you know how this might be happening?
