How can I change the Email Template/Settings?

Hi, i have set up a form and when i try to fill it out, the contents sent in the email is only the photo below.

the form should be like this

Hello,

Your notification email settings broke down. If you delete and rebuild this setting all fields in your form will be added in your e-mail template.

To delete and rebuild the notification mail settings,

1. Open the SETTINGS tab,

2. Click the EMAILS settings,

3. Click the Trash can to delete the notification.

4. Click the plus sign to create a new Notification Mail.

5. Click the NOTIFICATION EMAIL tab to create a new Notification Mail.

If you follow the above directions, you will see that all your form fields were added in the email template.
