Form not found. Account suspended.

Hello,

I am currently using a form named "Contrat de location" and I have a problem with it.

When I tried to go on the url, I've got an error message "Form not found, the form is disabled".

Can you please help me ?

Your forms was flagged by our reviewers team as phishing because you were asking for an ID number.

For forms asking this type of information, you need to comply with two things.

1. Add Terms of use widget (for your users to read and agree). This will surely make your forms not to be considered as guilty of phishing activities.

2. Add a Signature widget where your users sign and agree to provide such sensitive information.

We'll activate your account if you agree to comply with the above condition.

I don't understand why it was suspended, I just had a look at my form and there is no ID number asked.

Can you please check and tell me where is the problem ?
