In the past my account had been disabled due to suspected phishing because the email address was in front of a password field. I need a users email address so that I can send an auto response with the details they have submitted.
Could I have the email address at the end of the form to avoid my account being suspended.

The issue is not the e-mail address. It is the password fields. Unfortunately, we discourage users from the use of password fields on their form due to the high risk of phishing. Our platform does not support LOGIN forms and so our filters will flag most forms that requesta password.
May I know what is the purpose of the password?

The purpose of the password is to validate a users submission.
I could remove the password field and have s normal text field and. Label it as "Score Submission Code". World that be ok?

Yes. That should be OK and that is the more acceptable label/question for that field.
Please update your form now, and replace that fields label/question.
