I started looking around here on the forums and it appears that as soon as someone uses a "password" field, it blacklists that user. can someone show me a way other than to assign a password for someone to request a password... i will be using a simple .htpasswd and .htaccess file for users to log into my website with, so once i get a submission and I approve it, I would add them to the .htpasswd file. However I would like to give users the option to pick their own password. Im sure that this seems like an antiquated way to do this... but I dont plan on having more than 50 users on the site.

I have now re-activated your account and added your form to our white list to avoid this from occuring again. I believed there's nothing malicious with your form and your explanation is quite reasonable.
Thanks for using JotForm and apologies for the inconveniences caused. You should be able to login by now.
