Fields messed up (condition / do action)

The selected fields are descaled

Edit sumbmission (shows correct version)

Please check printscreens from Email-Confirmation and Submission report (same, wrong result)

The missing checkboxes on the Input Table field are caused by the injected custom CSS codes in the form — http://www.jotform.com/help/117-How-to-Inject-Custom-CSS-Codes
