I need whatever account this was charged from cancelled immediately and many months refunded. I have been trying to cancel since July.

As checked, your account is on a FREE subscription wherein monthly charges is not necessary.
If there's another Jotform account that you own, please let us know the username or the email address associated with it so we could process your cancelation request.
Or you may provide us a link to the form which was created from the said account so we could trace it.
