When you added a new field on your form , you have to edit the notification as well, then add the new field in your email notification.
To ensure that you will receive all fields on your form, please create a new notification. Please see this tutorial.
Note: you may also want to delete the old notification after you create a new notification to avoid duplicate notification.
Click Email alerts, Click Notification title (old), then click delete button.
