I wonder how I do that apareceçam titles also in the responses, that is, when I return the forms answered, because you see only the answers and so it is very confusing.

I don't mean to sound rude but your question is a little bit confusing. First, I do not see any "apareceçam" text in your form. Second, I do not get what you mean with "hidden in the back of the form answered".
Can you please elaborate more on your issue? Give us more detailed descriptions of your problem so we will be able to help you with the best of our ability.

Sorry but can you please rephrase the question? What does "apareceçam" mean exactly? I have used various translators without success.

In no time you were rude in your observation, I believe is that I do not know to express myself right, or perhaps the translator I'm using is not helping much.
Well, what happens is that the questions are not seen after returning filled in the email box, making it difficult at the time whether the answer is relative to any questions. For example:
The first question is: Given the questions competitive pricing, guaranteed supply, service and loyalty, point the top five companies in order of importance within each category: (THIS QUESTION IS THAT IS NOT SEEN WHEN THE ANSWERS AGAIN MAILBOX) - translate seen= ver/aparecer)

Okay, dendis. I think I now understand what you mean. Only the answers to questions are being displayed in the email. In other words, let's say a form had three (3) fields: "Name", "Email" and "Comment", and those fields were completed by the user as shown below:
However, the email notifying you of the submission contains John Smith, email@example.com and This is a simple example. but the labels (Name, Email and Comment) are missing. Correct?
If this is the case, perhaps it would be best to delete the notification and replace it with a new one. Please follow these instructions:1. Open your form on the Form Builder
2. Click on "Setup and Embed" tab,
3. Click on "Email Alerts",
4. Select "Notification",
5. Click on "Delete Email" button,
6. Click on "Email Alerts" again,
7. Click on "Add New Email" and complete the wizard.Please let us know if this worked.Thanks.
