I have been editing my form and although it looks okay, it is not passing all the information through in the email. Only getting name and email! Other fields are blank even though every field is required.
Here is a sample of a form I just tried as a test....
Question Answer First Name Last Name Street Address City State/Province Zip or Postal Code Phone email@example.com Your Event Date 04-15-2013 5:10 PM Type of Coverage wanted Your Budget Location Are there separate locations for your ceremony and reception?
