See my post below. I tried to follow the instructions on the help pages for GoDaddy but all that ended up on GoDaddy was the first page of a multiple page form. What can I do to get the whole form on GoDaddy?

The first page of the form shows up on GoDaddy but not the other two. I put
the GoDaddy website on preview to try to test the form. The first item on
the application is "I have read and agree to accept the vendor guidelines
listed on the Stilwell Kiwanis website at
http://www.strawberrycapital.com/vendors." This item does not show up on
the form on GoDaddy. This may be why I can't see pages 2 and 3 when I fill
out the form in preview mode but I don't know what to do to put it back on
the form. It shows on the JotForm preview. When I try to fill out the
form on GoDaddy a line appears at the bottom in red type but it flashes so
fast I can't read it. It may be saying that I need to answer the "I have
read..." item.

If you're referring to this form: https://www.jotform.com/93332879339975 I just tried cloning it and embedded it to my test GoDaddy site. However, I wasn't able to replicate the issue. The form works fine on my end. Here's the site:
Also, I'm not able to find an embedded form on this URL you mentioned: http://www.strawberrycapital.com/vendors
Could you share with us the link to the page where the form is embedded so we could also check?

It is due to the terms and conditions widget that you have in your form. Unfortunately, it doesn't render properly when embedded on your GoDaddy site.
Try this workaround to overcome the issue. Instead of using this widget, replace it with a regular checkbox field (Multiple Choice). This shouldn't have any problem.
Example:
Alternatively, you could also try re-embedding your form using its iframe embed code, this might work without any problem with the widget.

stilwellkiwanisAnswered on December 02, 2019 08:50 PMI did as you suggested. But when I was trying to figure out how to delete
the unused options, everything on the form that used more than the first 25
or 30 spaces made a column instead of continuing across the page. How can
I fix this. So I haven't tried to embed the corrected form on GoDaddy
until I can fix my mistake. I have no idea what I did to cause this.

the correct email address and the thank you went to my personal email which
I put on the test form. The only problem is in the print form on the
application. I pressed it before I pressed "Submit" but nothing happened.
What do I need to do fix this?
